What is Care Compass ?

Care Compass is used to help prevent care gaps within the healthcare system.  Our trained clinical staff is here to assist you along all walks of your wellness journey. We offer multiple services to help augment your current care.  Some of these services include remote patient monitoring, medication counseling, principle and chronic care management, nutrition and exercise management, and even assistance with making appointments!

What is remote patient monitoring and how does it work?

Remote patient monitoring is where ivira can assist your provider with monitoring your vitals form the comfort of your own home. Common monitoring programs include blood pressure, weight, and blood sugar.  Once enrolled in the program, patients are provided with a smart device where they can check their vitals at home and these readings will automatically be sent to ivira.  Ivira then monitors these readings 24/7 and reaches out if the patient ever has a critically high or low reading within 24 hours as well as communicates the readings with the patient on a monthly basis and with their provider.  This allows the patient’s provider to have an abundance of information at appointments to help make the most informed decision.

What happens if I have a really uncontrolled reading?

Our team is staffed with clinical pharmacists and nurses who are constantly monitoring your readings.  Should you ever have a very uncontrolled reading, our pharmacist or nurse will call you within 24 hours to assist.  Our team will also work with your provider should your readings be uncontrolled to either get you into the office sooner or even make adjustments over the phone! Every patient is assigned a pharmacist and a care compass specialist when enrolled and you are provided their direct line at enrollment, meaning you can call or text your clinical team at your convenience!

How often do I need to check my vitals?

We request that you check your vitals AT LEAST 16days per month in order to provide our clinical team and your provider with an adequate amount of readings.  However, we encourage checking even more regularly (there is not limit to how often you can check), since someone is always monitoring your readings so we are then able to catch any uncontrolled readings.

How do the smart devices send my readings to ivira and my provider?

Our smart devices are cellularly activated, meaning they transmit data just like a cell phone. You do not need internet or a cell phone for this service to work, just be in an area with cell reception (example: a place where you can normally make and receive calls on a cellphone).  The devices are extremely simple to use and all you need to do is check your reading and the machine will automatically upload the information to ivira in real time.  Ivira also shares your readings with your provider so they are also informed as to how you are doing.

How much does this program cost?

This program is covered by most insurance providers and is typically at no cost to you. Our team would be happy to look into your insurance and let you know if there would be any out of pocket cost to you.

How do I enroll in the program?

You can enroll in our program by calling our number (302) 274-0020 and letting us know you are interested in enrolling in remote patient monitoring.  Our team will then collect your information, process your insurance, and walk you throughout the entire enrollment process.  Enrollment typically takes 5 minutes, and you will receive your device within 5 business days of enrolling.
